Connecting Families to NH Healthy Kids - Tips for Homeless Shelters
Applying for coverage can feel overwhelming for families who are in transition and may have limited access to documentation and needed verifications. Staff at homeless shelters have the opportunity to make the process easier. Here are a few ways you can help the families you are working with:
Screen for health insurance status on intake forms
Provide address verification for families without a permanent home by writing a letter on agency letterhead attesting to their status and verifying the address where they are staying
Call the NH Healthy Kids office at 1-877-464-2447 when a family is struggling to provide needed documents. Our office can help to verify many of the needed proofs such as proof of income, citizenship status, and proof of ID
Become an Authorized Representative for families facing extraordinary challenges, such as chronic homelessness. Becoming an Authorized Representative allows your agency to receive copies of the renewal notices and other important reminders for covered children – learn more information by talking to our office at 1-877-464-2447
Learn how to help a family complete an application for coverage (sign up for a HK 101 presentation below)
Help enrolled families to stay enrolled; ask families with enrolled kids when their renewal date is, and offer help completing the renewal forms
