Reporting Changes
Your must report changes that may affect your child’s enrollment such as:
- Address changes
- If you get married, divorced or change your name
- Any change in family size including the birth or adoption of child, someone moving in or out of your home
- Increases or decreases in income or change in jobs
- If you obtain other insurance for your child
- If you or your daughter is pregnant
