Reporting Changes

Your must report changes that may affect your child’s enrollment such as:

• Address changes
• If you get married, divorced or change your name
• Any change in family size including the birth or adoption of child, someone moving in or out of your home
• Increases or decreases in income or change in jobs
• If you obtain other insurance for your child
• If you or your daughter is pregnant
