Report Changes

You must report changes that may affect your child’s eligibility such as:

• If your address changes
• If you get married, divorced or change your name
• If there is a change in family size including the birth or adoption of a child or someone moving in or out of your home
• If your income increases or decreases or you change jobs
• If you obtain other insurance for your child
• If you or your daughter is pregnant
• If your telephone number changes

You must call NH Healthy Kids at 1-877-464-2447 to tell us if you have any of these changes within 10 days of the change. You may have to provide proof of these changes.
