Application Questions

What do I need to mail in with my application?

Please review the checklist on page 8 of the application for the documents you will need to include with your application.

How long will it take to receive a determination?

Once we receive your application, you will be notified within 45 days with a request for additional information or a determination. If you have any questions during that time, please call us at 1-877-464-2447.

Can Legal Guardians and Grandparents apply for children?

Yes, as a legal guardian or grandparent, you may apply for a child in your custody. Your income will not be counted, unless you have adopted the child. However, the child's income will be used to determine eligibility.

Can I apply for a child not living in my home?

In order for us to determine if your child is eligible, it is necessary for the application and supporting documents to reflect information about the household in which the child primarily lives. If your child is uninsured, we encourage the custodial parent to submit a new application with the required information.

Can I fax my application and documents to NH Healthy Kids?

Yes, your signed application and supporting documents can be faxed to 603-228-8940. However, we may need to request that original documents be mailed to us.
