Important Change
Effective October 1, 2008

The federal government requires any children or pregnant women applying for Healthy Kids Gold or Medical Coverage for Pregnant Women to provide original documents to prove identity and citizenship.

There are a number of ways to provide these documents for verification:

1.  Mail the original documents to NH Healthy Kids, 1 Pillsbury Street, Suite 300, Concord, NH 03301-3556.  Once your originals have been verified, we will return them via mail to you within 5-7 business days.

2.  Bring your original documents to the NH Healthy Kids office in Concord and we will make a copy of them.

3.  If you do not want to mail your original documents to us or if you are unable to come to the office in Concord, please call us toll-free at 1-877-464-2447 for additional options.
